Become a Volunteer!

Thank you for supporting our school district! There are four volunteer levels:

• Levels 3 & 4*: Most flexible – can work with students unsupervised.

• Level 4: Only level permitted to drive students (essential for band trips).

• Levels 1 & 2: Many roles available too.

*Level 4 volunteers are especially needed

First-Time SLOHS Volunteers

• Download the Volunteer Packet from the link above.

• Submit hardcopies of completed paperwork to Heather Diesta at least 1 week before volunteering.

• If fingerprinting is required, allow 3 weeks total before volunteering.

• Keep a copy (digital or paper) of everything submitted.

• Once submitted, email volunteer@slotigerband.org to be added to the Band list.

Required Documents by Level (Submit hardcopies + show ID)

• Level 1: Volunteer Application, Code of Conduct, Confidentiality Agreement

• Level 2: All Level 1 items + TB Clearance (valid 4 years)

• Level 3: All Level 2 items + fingerprints on file with SLCUSD

• Level 4: All Level 3 items + driver-specific forms (see below)

TB Clearance (Levels 2–4)

• SLO County Public Health (Johnson Ave): $20, call (805) 781-5500 for appt. In many cases they can clear someone without requiring a TB test.

• MedStop (Madonna Plaza): $16 walk-in TB skin test; return 48–72 hrs later.

Fingerprinting (Levels 3–4)

• Must be on file with San Luis Coastal USD.

  • Already fingerprinted? Note the date on the application.

• New to SLCUSD? Choose one:

  • SLOCOE: Call (805) 543-7732 for appt. Cost: $80. Use “District Consortium Live Scan Request” form.

  • UPS Store (Foothill or Broad): Call ahead for pricing. Use “Request for Live Scan Service” form.

  • *Note: We do not recommend using Cal Poly for fingerprinting, as it has been rejected in the past

Level 4 Driver Requirements

Submit the following in addition to Level 3 requirements:

• Instructions for Transporting Students (form in packet)

• Volunteer Driver Information form (in packet)

• DMV Authorization for Driver Record Release (in “DMV Form for Level 4” packet)

• DMV Driver’s Record printout – Get it online for $2

• Copy of:

  • Driver’s License (front only)

  • Insurance Card

  • First page of Auto Insurance Policy Declaration

Returning SLOHS Parent Volunteers

Returning SLOHS Level 1, 2, and 3 Volunteers: Good news! Once you are cleared to volunteer at SLOHS, if you are a Level 1, 2, or 3 volunteer the only thing you have to do is provide a new TB clearance if your current one will pass the 4-year mark after July 1st. You do not need to turn in new forms each year as we were instructed to do in the past. If you need a new TB clearance, see above for TB clearance information and email a scan (or turn in a hardcopy of) your updated TB clearance to hdiesta@slcusd.org

Returning SLOHS Level 4 Drivers: Update TB clearance if required (same as returning Level 1, 2, 3 volunteers) Provide (by email is OK to hdiesta@slcusd.org) copy of this year’s Insurance Card. If Driver’s License has renewed the last year, provide copy.  If Auto Insurance company has changed since last year, provide first page of Policy.
